Bridge Port Marina will not haul, nor begin work until this form is filled out signed and returned. All materials are extra unless otherwise noted. All applicable taxes and environmental fees are extra. LOA includes; pulpit and swim platform. Payment is due in full upon completion of work. 2% per month charge on all accounts not paid within 30 days. I hereby authorize the above indicated work to be completed along with all necessary parts and supplies, and agree Bridge Port is released from liability, damage, or loss no matter how it occurs to the boat, motor, or equipment under their control and direction, and agree that the above property is insured by me/us against any damage or loss while on premises.
